Our organisation is led by Mental Health and Clinical Practitioners with over 20 years of experience across mental health, acute medical care, emergency response, and intensive care (ICU).
We support participants with complex mental health needs, behaviours of concern, and forensic backgrounds to live safely and meaningfully in the community.
Our supports are guided by comprehensive mental health and psychiatric assessments, formal risk assessments, and individualised care plans tailored to each participant.
Our support workers are experienced in managing complex participants and receive specialised, ongoing training, clinical supervision, and behaviour-aware education.
We provide nurse-led supports including chronic disease management, tracheostomy care, wound care management, and continence assessments conducted by a Clinical Nurse Consultant.
All supports are delivered in line with NDIS Quality and Safeguards requirements, with strong clinical governance and a focus on dignity, safety, and least-restrictive practice.

At Forever Care Disability Services, our purpose is simple — to provide safe, ethical, and high-quality support to people with complex needs, delivered with dignity, compassion, and clinical integrity.
As Managing Director, I am proud to lead a service that is clinically driven and people focused. Our organisation was established in response to a clear gap in the sector: the need for providers who can confidently and safely support individuals with complex mental health, behavioural, forensic, and high clinical needs in the community.
Our services are guided by experienced Mental Health Practitioners and Clinical Practitioners, with strong clinical governance underpinning every aspect of care. We believe that quality outcomes are achieved through comprehensive assessment, clear risk management, skilled staffing, and genuine collaboration with participants, families, support coordinators, and health professionals.
At Forever Care, we are committed to upholding the principles of the NDIS — choice, control, dignity, and safety — while delivering supports that are practical, responsive, and grounded in evidence-based practice. We invest heavily in our workforce through ongoing training, supervision, and professional development to ensure our team is equipped to meet the needs of those we support.
